After reviewing the monitor Attwell decided to send off Ziyech, before reversing the call seconds later.
He reverted to his original punishment of a yellow card for Ziyech but sparked confusion as the Moroccan appeared to receive two bookings without being sent off.
But replays clarified Kai Havertz was subject to the first yellow card, allowing his team-mate to remain on the pitch.
